Abstract

|
The introduction of Prostate-specific antigen screening has resulted in a significant stage migration of prostate cancer, with the incidence of nodal metastasis decreasing from 20% to 40% in the 1980s to 5% to 10% more recently. Lymph node metastasis is generally regarded as a poor prognostic indicator in patients with prostate cancer. Although androgen deprivation therapy has been shown to benefit patients with high-risk localized, or locally advanced prostate cancer who are undergoing radiation therapy, its role in patients with nodal metastasis after surgery has not yet been clarified. Although immediate adjuvant androgen deprivation therapy has been found to improve OS and progression-free survival in patients with nodal metastasis after surgery, controversies remain about appropriate timing and duration of adjuvant hormonal therapy. In this article, we reviewed the outcomes of various treatment options for node-positive prostate cancer and addressed the role and appropriate timing of adjuvant hormonal therapy.
